By completing this personal release form, you agree to be photographed and recorded by the Australian Education Union for use in a production (“Works”).

You declare that any recorded testimonial is true and correct to the best of your knowledge at the time of its recording and reflects your personal beliefs and experiences. You understand this release will permit the AEU to proceed with the Works which include but are not limited to:

– Videography

– Photography

– Sound recording

– Editing

You agree to waive all personal rights and objections to the Australian Education Union and its affiliates for the use of your voice, image or name for any and all broadcast purposes and performances thereof, accompanied by any narration and dialogue whatsoever, and publicity in connection with, and/or for any other trade and advertising purposes. In consideration of the above release, you agree to forego any fee in connection with the Works .
